To request your medical records, please call 765-662-4792, extension 2, Monday through Friday, 8 a.m. to 4 p.m. or e-mail
medicalrecords@mgh.net. The location for pick up of records is 500 N. Wabash Ave., Suite 112. Please bring a photo ID with you, it is required to receive your medical records. 

For your convenience, we have posted the authorization form here for you to print, fill out and also bring with you. The form allows us to process your records in a more timely manner.
